Collection forms
Get reviews you can actually use
A review is worth more when you know which product it is about and whether you may quote it. One form collects the review, sorts it by product and asks the few questions that make it credible.
Free plan included. No credit card.

Why the form matters as much as the review
Sorting
The right review on the right product page
Reviews arrive tagged by product. No sorting by hand, no form per product.
Credibility
Context that makes a quote believable
"Great service, two years a customer" beats "great service". Ask for the detail once.
Permission
Consent captured with the review
A "you may quote me" checkbox travels with the testimonial, so nobody has to ask later.
One form, every product
Ask which product the customer bought and give each option a tag. Reviews arrive sorted, and the widget on each product page shows only its own.
- Dropdown options map to your workspace tags
- The form's default tags still apply
- Widgets, Wall of Love and exports filter by tag

Every answer, summarised per question
Answers to your questions are stored with the review, included in the CSV and summarised on the form's analytics page. They never show on widgets.
- Short text, long text, dropdown, checkbox and NPS fields
- Required or optional, with your own placeholder
- Two custom fields on free, ten on Pro

A review and a score from the same customer
Add the recommendation question as a field and the form analytics report an NPS from people who also left a review. For NPS from visitors who will never write a review, use the NPS survey.

Questions
Why not use Google Forms or a survey tool?
Because the review then sits in a spreadsheet. A Shapo form puts it in your dashboard for approval, tags it by product, and it can be in a widget on your site the same day, with the reviewer's photo and details.
What can a form collect?
A star rating, a written testimonial, up to three photos or a video, and the reviewer's name, email, job title, company, website and photo. On top of that, your own questions: short text, long text, dropdown, checkbox or a 0 to 10 NPS scale.
Can one form serve several products?
Yes. Add a dropdown like "Which product did you buy?" and give each option a tag. A widget filtered on that tag shows only that product's reviews.
Are the answers shown on my widgets?
No. Custom field answers stay in your workspace: on the testimonial, in the CSV export and on the form analytics page.
How many custom fields can I add?
Two on the free plan, ten on Pro. Forms themselves are unlimited on every plan.
